Cancer: moonstone
Cancer is one of the strongest agreements in the whole birthstone scheme. Western lists give moonstone or pearl, the June and July month lists give pearl and moonstone, and Jyotish gives the Moon's gem, which is pearl. Three systems, one answer, because all three connect this part of the year to the Moon.

The stone most often assigned
Moonstone leads most Cancer lists, with pearl close behind and often given equal weight.
Moonstone is orthoclase feldspar with microscopic albite layers that scatter blue light, producing the floating sheen called adularescence. In Indian tradition it is chandrakanta, moon-formed, said to be made of solidified moonbeams, and it is traditionally given to lovers and to travellers.
That Indian tradition is genuinely old and independent of the Western zodiac lists, which makes moonstone unusually well supported as a lunar stone. Roman writers reached the same conclusion, with Pliny recording the belief that the stone's image waxed and waned with the moon.
At hardness 6 with two cleavage directions, it is a pendant stone rather than a working ring stone.
Where the assignment comes from
The lunar logic here is the oldest and most consistent in the zodiac scheme. The Moon rules Cancer in both Western and Vedic astrology, and the Moon's stones have been pearl and moonstone in both traditions for as long as either has been recorded.
That agreement exists because the underlying reasoning is the same in both places: a pale, translucent, shifting stone resembles the moon, and both cultures noticed.
Cancer also overlaps June and July, which supplies pearl from the month list, so someone born in the first half of Cancer is given the same answer from three directions.
The Vedic alternative
In Jyotish, Cancer is Karka, ruled by Chandra, the Moon, whose gem is pearl, mukta. It is prescribed for emotional steadiness, for the mother, and for the mind in its receptive aspect, and it is one of the gentler prescriptions in the navaratna, less hedged with warnings than blue sapphire or red coral.
Almost all pearls sold today are cultured, meaning nacre laid down by a living mollusc around a deliberately introduced nucleus, and that is normal rather than a substitute. Vedic practice generally accepts natural or cultured pearl, and moonstone is sometimes accepted as a substitute where pearl is unaffordable.
Pearl is also the most fragile gem in the catalogue: perfume, hairspray, vinegar and sweat all etch it.
Other stones cited for this sign
Pearl is the co-primary rather than an alternative, and it is the Vedic answer.
Ruby appears in some Cancer lists borrowed from July, and emerald appears occasionally with no clear reasoning.
Selenite is a modern addition on the strength of its name, from Selene, the moon, and it is worth knowing that this is a naming coincidence rather than a tradition. Selenite is also gypsum at hardness 2 and dissolves in water, so it is a poor thing to carry.
Of the practical options, moonstone in a pendant is the best-supported choice, and a small agate or clear quartz is the sensible thing to actually carry.
